Cervical Radiculopathy

What Is Cervical Radiculopathy?


Cervical radiculopathy occurs when a nerve root in the neck becomes irritated or compressed. The cervical spine contains nerves that travel from the neck into the shoulder, arm, forearm, and hand. When one of these nerves becomes irritated, symptoms may be felt locally in the neck or may travel into the upper limb.


This condition is often referred to by patients as a pinched nerve in the neck, nerve pain in the arm, neck pain with tingling, or neck pain radiating down the arm. Symptoms may vary depending on which nerve root is affected and how irritated the nerve is.


Cervical radiculopathy may develop gradually from joint or disc-related changes, or it may occur after a flare-up involving neck strain, posture, lifting, repetitive work, or prolonged desk and device use. Some individuals experience sharp pain, while others notice numbness, tingling, burning, electric-like sensations, weakness, or reduced grip strength.


Diagnosis should be confirmed by a licensed medical professional, especially when symptoms include significant weakness, progressive numbness, worsening pain, trauma, or changes in coordination.


Individuals May Experience


  • Neck pain that radiates into the shoulder or arm

  • Tingling or numbness in the arm, hand, or fingers

  • Burning, sharp, or electric-like pain down the arm

  • Weakness in the arm, shoulder, or grip strength

  • Pain that worsens with certain neck movements

  • Symptoms that change with posture or head position

  • Shoulder blade pain or upper back discomfort

  • Tightness in the neck, upper trapezius, or chest

  • Difficulty sitting at a desk or using a computer

  • Reduced tolerance for driving, lifting, or overhead work

  • Discomfort that travels from the neck into the forearm or hand

  • Protective muscle guarding around the neck and shoulders


More urgent medical assessment may be required if symptoms are worsening, involve significant or progressive weakness, affect walking or coordination, follow trauma, or are associated with severe unexplained pain.


What Contributes to Cervical Radiculopathy?


Several factors may contribute to cervical nerve irritation or increased mechanical stress through the neck and upper body, including:


  • Disc changes in the cervical spine

  • Joint degeneration or arthritic changes

  • Narrowing around the nerve pathways

  • Prolonged forward head posture

  • Repetitive neck strain

  • Desk work, screen use, or sustained sitting positions

  • Reduced thoracic spine mobility

  • Rib or clavicle mobility restrictions

  • Muscle imbalances in the neck, shoulders, and upper back

  • Poor scapular control or shoulder mechanics

  • Previous neck, shoulder, or upper back injury

  • Protective muscle guarding around irritated tissues


Often, nerve irritation is influenced by both local and whole-body mechanical stress patterns. For example, reduced thoracic mobility may increase strain through the neck. Tightness in the chest or upper shoulders may alter shoulder blade position. Poor desk posture may increase cervical loading and contribute to symptoms that travel into the arm.


Cervical radiculopathy should be properly assessed because similar symptoms can come from different sources, including nerve irritation, shoulder conditions, peripheral nerve entrapments, or other medical concerns.


How Manual Therapy May Help


Manual therapy and rehabilitation may help improve joint mobility, reduce secondary tension patterns, and support better movement around irritated nerves. The goal is not to “force” the nerve or aggressively manipulate the neck, but to reduce mechanical stress and improve the environment around the cervical spine, upper back, ribs, and shoulder girdle.


Care should be tailored based on symptom severity, irritability, medical history, and whether symptoms are primarily pain-related, mobility-related, or associated with weakness or neurological changes.


Osteopathic Manual Therapy


Osteopathic Manual Therapy may focus on how the neck functions in relation to the upper back, ribs, clavicles, shoulders, and posture. When the thoracic spine, rib cage, or shoulder girdle is restricted, the cervical spine may absorb more strain during daily movement, desk work, lifting, or reaching.

Physiotherapy


Physiotherapy may help support cervical radiculopathy by improving neck strength, postural control, nerve mobility, shoulder stability, and movement tolerance. This is especially useful when symptoms are aggravated by desk work, device use, lifting, driving, or repetitive upper body activity.


Physiotherapy rehabilitation may involve:


  • Nerve mobility or nerve gliding exercises when appropriate

  • Strengthening of deep neck stabilizing muscles

  • Postural retraining for desk and device use

  • Scapular stabilization exercises

  • Thoracic mobility exercises
