What Are Muscle Spasms?


Muscle spasms occur when muscles contract involuntarily and remain tightened for a short period of time. They may feel like a sudden cramp, gripping sensation, knot, sharp tightening, or painful restriction in the affected muscle.


Muscle spasms can occur in many areas of the body, including the lower back, neck, shoulders, calves, hamstrings, hips, feet, and upper back. Some spasms happen suddenly during exercise or movement, while others develop gradually due to fatigue, strain, poor posture, dehydration, or protective guarding around an irritated joint or injured tissue.


A muscle spasm is often the body’s way of protecting an area. If a joint is irritated, a muscle is fatigued, or the body senses instability, nearby muscles may tighten to limit movement. This can be useful temporarily, but if the spasm continues or keeps returning, it may restrict mobility, increase pain, and create compensation patterns in surrounding areas.


For example, a lower back spasm may cause the hips, pelvis, and upper back to stiffen. A calf spasm may change walking mechanics. A neck spasm may increase shoulder tension and limit head rotation. Understanding why the muscle is spasming is often more important than only trying to relax the muscle.

Electrolytes such as sodium, potassium, calcium, and magnesium play a role in muscle contraction and relaxation. When hydration or electrolyte balance is disrupted, some individuals may be more prone to cramping, especially during exercise, sweating, heat exposure, or prolonged physical activity.


However, not all muscle spasms are caused by dehydration or salt imbalance. Many are related to mechanical factors such as joint restriction, muscle fatigue, poor movement patterns, lack of strength, or protective guarding. This is why recurring spasms should be assessed from both a movement and lifestyle perspective.


How Manual Therapy May Help
